This form will assist the club in providing necessary information to COVID 19 contact tracing authorities if needed.  Each participant must complete this form before participation will be permitted.

It is important to complete the form accurately and honestly.  Failure to do so has the potential to put other people at serious risk of infection.

All players, coaches, volunteers and spectators must undergo a self-assessment for any COVID-19 symptoms prior to every session. No-one should leave home to participate in sport if they, or someone they live with, has symptoms of COVID -19 currently recognised as any of the following:
-- A HIGH TEMPERATURE
-- A NEW, CONTINUOUS COUGH
-- A LOSS OF, OR CHANGE TO, THEIR SENSE OF SMELL OR TASTE

Should an individual have demonstrated any such symptoms, they must follow NHS and PHE guidance on self-isolation: https://www.nhs.uk/conditions/coronavirus-covid-19/symptoms/

You must NOT attend if you have been instructed by contact tracing authorities (NHS Test and Trace / Test and Protect / Test, Trace, Protect) in your location to self isolate for a period covering the date of this training session.
