The News Patient Choice Award
The News is inviting readers to nominate a Portsmouth Hospitals University NHS Trust employee or team for The News Patient Choice Award in this year's Pride of Portsmouth Awards.

The criteria includes:

- Has made a real difference to that patient

- Has gone the extra mile to provide an exceptional service for patients, their relatives and carers

- Has delivered an excellent quality of care
