School Numbers Update Form

Please fill out this form anytime you have an update to the number of students participating in Feeding Our Kids at your school. This includes children receiving home delivery or picking up from district meal distribution sites.

If you are signing up a family for pick-up at a meal distribution site or for home delivery, and there are siblings at other schools, make sure you coordinate with the other school's social worker or program coordinator so that the family isn't double counted.

If an increase is requested of more than 5 bags or 20% of your previous number (whichever is greater), the FOK Board of Directors needs to approve the request. Also, if more than 2 increases are requested per quarter, the FOK Board of Directors will need to review additional requests.

If the total number of students across all schools is already at the budgeted amount, increase requests cannot be accepted until (1) the Board of Directors approves an increase in the overall number or (2) other schools decrease their numbers.
