I am participating in workout and fitness classes, programs, workshops, and events titled Wind Down at La Plazoleta, Popular Center, hosted by Modaqui, Inc and Banco Popular Puerto Rico, scheduled for October 29th, 2024 ("the Event"). These sessions may include physical activities that could be strenuous and potentially lead to severe physical injury or death. I acknowledge these risks and affirm that I have consulted with a physician regarding my participation in the Event. I am physically fit and free of any medical conditions that might affect my ability to safely participate. I accept full responsibility for any risks, injuries, or damages that may occur during the Event. I agree to inform the instructor of any physical limitations or discomfort I experience and accept responsibility for my actions and limitations. In further consideration of being allowed to participate, I voluntarily waive any claim against Modaqui, Inc., its instructors, contractors, directors, partners, owners, and employees for any loss, theft, or damage to personal property, or for any personal injuries or damages incurred at the Event or associated facilities. I also release and forever discharge Modaqui, Inc. from any claims resulting from negligence or other actions at the Event. I grant Modaqui, Inc. the right to photograph and record me and my property during the Event, and to use these photographs and recordings for publicity, illustration, advertising, and web content without compensation. I waive any rights to payments for the use of such materials. *
