This information is collected to help determine how many children ages 1-18 will pick up sack lunches June 1st thru June 30th, or while participation lasts. We will be providing breakfast and lunch this year. It is not required to enter information, but the information helps create an accurate count. Please fill out as soon as possible so we can continue planning. Children are not required to attend CCE to pick up the sack lunch.  Thanks for filling out the form.
