This registration form is for adult Catholics (baptized as an infant in the Catholic Church) who wish to complete their Sacraments of Initiation, Confirmation and if necessary, First Holy Communion.
Adults who desire to join the Catholic Faith, be Baptized, receive First Holy Communion and Confirmation should register with the RCIA program. Please view the RCIA page under Sacraments & Liturgy for more information.
