Saint Augustine Church Indoor and Outdoor Public Mass Attendance Sign-in for Contact Tracing During COVID-19 Public Health Crisis  
This form is NOT intended for RSVP.  This form is used to collect names and contact information of those attending Indoor or Outdoor Public Mass Services for the sole purpose of assisting the process of Contact Tracing, in the case of a Coronavirus outbreak.  Please submit one form per household.  

We are currently allowed 25% capacity or maximum of 100 people (whichever is less) per indoor Mass Service. Indoor seating will be offered on a FIRST COME- FIRST SERVE basis until the maximum 100 capacity is reached.
