Dear Candidate,

You have decided to embark upon an incredible and exciting journey! The Academy of Life Sciences will provide you with an academic foundation and technical skills that will enable you to continue your education beyond high school, leading to a desirable health-related career.  This is a unique opportunity for you to participate in a small, career‐focused, learning community with other students who are interested in many of the same subjects. Applications from students to attend the Academy of Life Sciences will only be processed if the student is assigned to East Wake High School as the base school.

Through the academy, you will also learn skills such as communication, professionalism, legal responsibility, ethics, safety practices, and interpersonal dynamics. Please submit the application by Friday June 26th

Students will be notified of acceptance via email.

PRE-MEDICAL TRACK REQUIRED ELECTIVES:
9th grade:  Foundations of Health Science (Health Team Relations) (Paired with English 1)

10th grade:  Health Science I (Paired with Biology)

11th grade:  Health Science II or Biomedical Technology (Paired with English 3)

12th grade choose ONE course:  Nursing Fundamentals and Practicum (CNA) and/or Pharmacy Technician (Paired with Economics & Personal Finance)

11th or 12th grade:  Required Internships (135 hours).  This can be done during the summer

PRE-MEDICAL TRACK SUPPLEMENTAL ELECTIVES:
Microsoft Word & Powerpoint, Career Management, approved Career & College Promise (CCP) CTE Pathway Courses
10th grade: Biomedical Technology I
11th grade: Biomedical Technology II, Anatomy & Physiology (currently offered through NCVPS)
12th grade: CTE Advanced Studies

MEMORANDUM OF UNDERSTANDING
As a member or candidate for the Academy of Life Sciences at East Wake High School, I acknowledge and understand that:

* I am making a commitment to participate in the required academy courses (see details at beginning of application).
 
* I must maintain academic excellence while enrolled in the Academy. This includes maintaining a cumulative GPA (weighted) of a 2.5 by the end of the sophomore year and through graduation. Dismissal from the program may result if my GPA falls below this standard. (One semester period of academic probation is allowed before removal from the Academy.)

* I must report to Academy activities and job shadowing opportunities dressed appropriately in business attire and/or approved dress.

* Regular updates will be sent home via email to both the Academy member and parent/guardian. It is necessary for me to have a valid email address on file. I will notify the Academy Coordinator of any changes to my contact information (email and/or phone number).

* Dismissal from the program may result from continuing disciplinary problems with any teacher or administrator (including teachers and administrators not affiliated with the Academy) after a suspension from school.

* Major disciplinary incidents will result in automatic dismissal from the Academy.

* Participation and/or support of scheduled academy meetings/activities is expected of all members. My participation will be documented in the work-based learning portfolio that will be maintained throughout my Academy experience.

* A 135 hour internship course must be completed before graduation. This will be for academic credit and should be completed the summer between the junior and senior year or senior year. Transportation to internship is required.

* Some internship sponsors request interns complete Nursing Fundamentals and earn a CNA prior to the CTE Internship.

* Students in the Pre-Medical Track will earn their CPR Certification and OSHA 10-hour Industry Certification for Healthcare.
