Copy the address (link) after you have submitted your form. This will allow you to edit the form if you miss additional days this month.  

The employee must be ordered by the District, a medical professional, or the Arkansas Department of Health (ADH) to quarantine or isolate due to COVID-19 to receive leave.

You will only need to provide one proof for days that are taken simultaneously. (excluding weekends)
