Ryle Band Instrument Check-Out
Fill this out while you are checking out a school-owned instrument. If possible, fill this out with a parent/guardian. Band instruments will be available for check-out on Thursday, August 20th from 4:30-6:30 pm. Each instrument rental is $30, which can be paid through cash, check, or online on the Ryle Band website. Please pull up to the side of the band room and remain in your vehicle during the check-out process.

Please read the following statement: The undersigned pupil and his or her parent and/or legal guardian agree to pay the sum of THIRTY DOLLARS ($30.00) as rental for the use of the instrument for the agreed upon period of time. If a second instrument is needed, an additional $30.00 will be charged for the second instrument as well. It is further agreed that if the instrument is damaged due to any cause, with the exception of wear from careful use, then we agree to pay the amount of the repair bill for the instrument, and if the same is damaged beyond repair, then we agree to pay the above stipulated agreed value of the instrument. We further agree that no person, other than the undersigned pupil will be allowed to handle or use this instrument without the consent of the Director. We agree to surrender the above instrument and accessories immediately after the expiration of this rental agreement or earlier upon the request of the Director or proper school officials. This agreement covers the period of 2020-2021 school year. *
