(If you are a parent/guardian completing this form for your child, please indicate so. If you are a teen interested in joining, because we take safety seriously, please ask your parent/guardian to complete it on your behalf or provide their email and we will contact them to complete it on your behalf.)

We want to make sure parents/guardians are completing this interest form and thus are agreeing for their child to participate on in an online Dungeons & Dragons campaign. Please tick the appropriate box below and we will email you to supply some kind of confirmation that you are the parent/guardian of such a child.
We are, out of necessity, collecting email addresses. We would love to include you on future communications about our programs and services but would only do so with your express permission. Please indicate your preference below.

We will have an introductory session (called a session 0) supervised by an adult with appropriate clearances to work with kids to discuss online safety. Thereafter, another teen who has volunteered to serve as Dungeon Master will run the game. Let us know if you'd like to be present at this first session as an observer.

In addition, please let us know here if there are any topics that should be off limits in the game (e.g., phobias, triggers/activations). We plan to follow GameOn!'s general policy about kids games, which includes: PG rated content; no curse words or other foul language; no murder or killing of children, innocents, or fellow humanoids; no content that would be included at a PG-13 or above movie.
