I have read about and understand the physical requirements for this meetup either in the meetup description or this form. I am confirming that I am physically capable of doing this meetup. I will be solely liable for any risk resulting from me not being physically capable of doing this meetup. You will need to be able to physically pry off mussels. *
I understand what gear/equipment is needed for this event and will have that gear/equipment ready for this meetup. If I don't have all my equipment then any risks arising from that will solely be liable to myself. *
I understand that organizers, hosts, and other members aren't professionally trained guide services and should not be held accountable as such.  *
I am responsible for my own safety. I have read about and assumed all risks that could arise. *
I am aware there is a risk of shellfish poisoning even though WA DOH cleared the beach we are going to for harvest. I am aware they are human and cannot be 100% accurate. Any harm resulting from any poisoning falls solely upon myself *
I am aware that there is a risk of tides. There's a low chance but possible chance I could be stranded due to rising tides. I will take full responsibility for making sure that I am aware of any tidal dangers and any failure to do so falls solely upon myself. *
I am aware that beaches and shellfish might have sharp objects that could injure myself. Any injury resulting from them rests solely on me.

I acknowledge that I have read the foregoing and fully understand the potential dangers incidental to engaging in this meetup , and I am fully aware of the legal consequence in submitting this instrument, and I understand and agree that this document is legally binding and will preclude me from recovering monetary damages from any Meetup members and organizations for personal injury, property damage, or wrongful death caused by the negligence, gross or otherwise, of the released parties, whether passive or active. The aforementioned Acknowledgment of Risk / Waiver of Liability shall continue in full force and effect until such time as revoked, in writing, by the undersigned, without limitation as to its duration. The terms and conditions herein are intended to be as broad and inclusive as permitted by the laws of the State of Washington , and if any portion of this document is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. The undersigned further acknowledges that I am authorized to execute this Acknowledgment of Risk, Waiver of Liability, Indemnification, and Consent.

I hereby acknowledge that it is my sole responsibility to provide for any medical expenses or other attendant costs which may be incurred as a result of any injuries sustained while participating in the activities with this meetup. I further acknowledge that I have adequate insurance to cover any injury or damage that I may suffer while participating in the activities and assume the risk of any medical or physical condition that I may have while participating in same. I acknowledge my obligation to defend, indemnify, and hold Organizers and Hosts and Meetup Members and Meetup Groups harmless as to any and all claims of any nature, including all costs, expenses, and attorneys’ fees, which may in any manner result from or arise out of the services provided by or on behalf of this meetup and related activities .
