DCCC Self-Reported Travel Form

In an effort to help keep our community safe, DCCC request all students, faculty and staff to report recent and upcoming domestic and international travel plans.

Any students, faculty, and staff returning from international travel or out-of-state travel since February 28, 2020, are required to complete the Self-Report Travel form here. Those returning from international trips AND out-of-state trips to areas of the U.S. with high coronavirus activity are required to take the necessary steps to self-quarantine for a minimum of 14 days from the date of return in order to monitor their health. Visit the CDC website for instructions for self-isolation and symptom monitoring. College employees should speak with their supervisors about appropriate leave and telework options during quarantine.

All nonessential college travel has been postponed or cancelled, including study abroad experiences, conferences and meetings.

The college strongly discourages personal out-of-state travel due to the health risks. Employees with travel planned for outside of North Carolina should talk with their supervisors immediately.
