Private Lesson Request Form

This form is to request Private Lessons for your non-competitive child over the age of 5. This program is a monthly commitment. Filling out this form does not guarantee your time slot, although we will do our best to accommodate every request in the order it is received. After your request is submitted, we will contact you to confirm time slot availability and start date. Please contact info@kzelitegymnastics.com with any questions.

Please tell us your desired time slot. *

Format your response DAY and TIME, i.e. MON 9:00 AM. 30-minute sessions are recommended for recreational gymnasts. If you would like a longer session, please make a note in your response.

Please tell us the name of the child who will be receiving Private Lessons. If there is a sibling living in the same household that will also attend, please list them as well (there is no additional cost for siblings in the same session). *

Please let us know what your spotting preferences are. *

I am comfortable with the coach spotting my gymnast as long as both are wearing masks.

I prefer the coach maintains physical distance at all times (no spotting).

Please confirm that you understand and agree to all adults and children over the age of 2 being required to wear masks at all times while in our gymnastics facility. *
