By signing up for the Water Day Clean-Up, I, as the volunteer or as an adult parent or a guardian of a child under 18 years of age who is registered as an Event participant, do hereby agree to waive and release for myself and/or my child (if applicable), any and all claims arising out of or in any way related to the Event that may come into existence against the City of Paterson, its elected and appointed officials, employees, volunteers, professionals and others working on behalf of the City. I hereby certify that I am the adult parent or guardian of the registered participant (hereinafter referred to as “Child”), a minor child under the age of eighteen years and consent to Child’s participation in the Water Day Clean-Up (hereinafter referred to as the “Event”). I understand and acknowledge that I am fully aware of and assume the risks, including but not limited to the risk of property loss or damage, personal injury or death, arising from or in any way related to Child’s participation in the Event.
