2020 LBH Week Request
FOR GROUP LEADERS ONLY!  Please fill out this form to request which week and an estimate of how many missionaries you think you will have. In order to accommodate for all weeks, we are asking parishes to consider two options.  After December 1st, we will contact you confirming you LBH week. We will try our hardest to accommodate for your first choice week.

This form will be available to Parish Leaders until December 1, 2020. Online liability forms will be made available after December 1st for individual students to register for their group's week.

**High School Missionaries:  9th - 12th grade students during the 2019 - 2020 school year.
**Middle School Missionaries:  6th - 8th grade students during the 2019 - 2020 school year.
