Please be aware that orders cannot be placed for individual households.
To order at-home rapid COVID-19 test kits, please fill out this form. Once submitted, the order will be reviewed, and you will be contacted for next steps.
Please note: You will be contacted once your order is approved to schedule a pickup day/time. All orders must be picked up at Public Health Services. We require at least 2 full business days to process orders. Thank you for your understanding.
