Please fill in ALL fields.  Put N/A if not applicable.  Once you fill out the form, you should receive a reservation contract within 72 hours that will need to be signed and returned ASAP to secure your seats.  Filling out THIS form does NOT reserve your seats.  Your seats are NOT reserved and secured until we receive the signed reservation contract back in our office.  Please pay close attention to the payment and headcount deadlines listed on the reservation contract.  If you create a payment and headcount deadline for your group on your own that is later (past) the deadline we list on your contract, we will only honor the deadline we assign.  If you need to cancel your group once you have signed and returned your contract, please let us know ASAP as we will have schools on a waiting list to get into the show.  Thanks.
