You are beginning the intake process for all new clients receiving financial coaching from me, Dylan L. Ross, CFP, AFC. I will also ask established clients to fill out these forms if they haven't had a financial coaching session with me in the over 12 months.

Filling out forms isn't fun, but I promise to do my best to keep the information collected to only what I truly consider useful, and essential.

Before you begin filling out this form, please be sure YOU AGREE AND CONSENT to these things:

1) SUBMITTING THIS FORM ELECTRONICALLY, AS WELL AS TYPING YOUR NAME AND CLICKING ON THE FINAL "SUBMIT" BUTTON AS YOUR ELECTRONIC SIGNATURE.

2) ENTERING YOUR EMAIL ADDRESS, AND RECEIVING A COPY OF ALL OF YOUR RESPONSES AT THAT EMAIL ADDRESS. This insures that the responses to this form connect with a valid person and address on the other side.

Here's a quick rundown of how this will work:

--> Provide some basic information about yourself (name, phone number, etc.)
--> Agree to my financial coaching terms, policies, and procedures (the legal stuff)
--> Fill me in on your reasons for seeking financial coaching
