The purpose of the following form is to have a credit card on file that we will charge if you do not fulfill your fundraising commitment or you do not show up for a volunteer commitment you signed up for. The payment for not fulfilling you fundraising commitment or not showing up to a volunteer commitment is 200.00 per spot.
Every effort will be made on our part to contact you and try and help you to fulfill your fundraising commitment before we process your credit card.
By filling out the following form you are giving us access to charge your credit card ONLY in the event that you do not fulfill your fundraising commitment or do not show up for a volunteer commitment that you signed up for.
If you need help in fulfilling your commitment please reach out to the bingo coordinator who has a list of people that you can hire to work your volunteer commitments.
