Please complete this form to apply for a place at a school run club for children in years 1 to 6.

You will need to complete a form for each child.

These clubs will take place on the school field (or Upper KS2 Playground)

Due to the popularity of these Clubs, please do not send your child unless you receive a confirmation email that your child has secured a place. In the event of oversubscription, children will be selected on a random basis, with priority given to those who have not previously attended.

I confirm I will encourage my child to remember their own change of kit (if required) which will be clearly labelled with their name and class. In the event the Club has to be cancelled due to poor weather, the school will endeavour to give as much notice as possible via schoolcomms text. In this event, you will need to collect your child promptly as there is no facility to accommodate them in the After School Club at this time.
