Students: DO NOT complete this form. The teacher in charge of science fair at your school must complete this form for all entrants from your school.

All personal details will be kept confidential and used only for fair purposes.

Please provide an email address you will regularly check around Fair time. This will be used for all communication, including final Fair details and instructions for students attending, and advising of any prizes that have been won. Please check you have entered your address accurately.
