MVES Take-Home Meals Enrollment Form
If you would like your child to take home meals for the days he/she is participating in eLearning, please complete this form.  Take-Home Meal orders are due before 10:00 am on the day your child attends school.  Once your child has enrolled, he/she will receive meals EACH WEEK on the day he/she attends school.  If you would like to discontinue the take-home meals program, please notify Kim Patterson by email before 10:00 on the day your child attends school.

4 breakfast entrees (1 frozen pancake, 1 frozen cinnamon roll, 1 frozen Belgian waffle, 1 frozen banana bread)
4 lunch entrees (1 frozen pasta bowl, 1 frozen three bean chili bowl, 1 frozen Alfredo bowl, 1 frozen Grand strand sandwich)
1 frozen roll
4 fresh apples
4 applesauce cups

* Additional components are available and will come together in a separate bag.
   You are NOT required to take them. See Below:
 
1 celery
1 baby carrots
1 garden salad
1 bean salad
1 - ½ gallon milk

Pick-up Information
Students comes to the Cafeteria when dismissed at the end of the school day and will pick-up meals in the serving line.
* By submitting this form, you are requesting to be enrolled in the Take-Home Meals program.  Your child will receive 4 take home breakfasts & 4 take home lunches each week.  
Full Pay Students are expected to pay: $10.00 per week
Reduced Price Students are expected to pay: $1.60 per week
Free Students receive meals at no cost. Breakfast is free for all students.
Please have money on your child's account or pay at time of service.
