Please complete the following to apply for your student.  Upon submission, the following documents need to be submitted to the school office before your application is complete and reviewed.
 --- A recent photo of the applicant
 --- A copy of the applicant's birth certificate
 --- A copy of passport with landing stamp (if child is born abroad)
 --- A copy of immunization card (front & back)
 --- Most recent school report
 --- Application fee of JM $5000 can be paid to The Montego Bay Christian Academy Scotiabank Business Acct #129518 branch 10215.  Please forward payment receipt to office at office@mobayca.org
