Please fill out completely for each student participating. There is no need to print this form - just "submit" at the bottom. Please go to
www.registermyathlete.com to submit the physical and medical paperwork. Physicals need to be completed by July 1. This form, as well as the first payment, is due by June 14 in order for student to attend Marching Band Summer Camp.
The first $400 payment is due to the Corona del Sol Bookstore by June 14. Checks made payable to "Corona del Sol" - include student name in the check memo line. Checks can be sent to CDS Band, 1001 E Knox Road, Tempe, AZ 85284, or turned into the bookstore in person during school hours (send receipts to the Band Office). This first fee can also be made with a TAX CREDIT donation to the band - send receipts to the Band Office.
The $50 Tempe Union Activities payment and other district-assessed course fees can be made at book distribution in Late July (after Camp).
