Special Instructions for Participation
Parents/guardians must complete this online participation form for their students to attend the event.  Students shall read and adhere to the guidelines set forth in the San Diego County Office of Educations (SDCOE) Administrative Regulation No. 6163, Student Use of Technological Resources (https://www.sdcoe.net/Board/Documents/ar/6163.pdf). Staff and Students shall also read and adhere to the guidelines set forth in SDCOE Administrative Regulation No. 4046, Staff and Student Virtual Interactions (https://www.sdcoe.net/Board/Documents/ar/4046.pdf).
IMPORTANT PARENT ADVISORY AND INFORMATION – PLEASE READ:
The purpose of the event is to bring awareness to careers in culinary and hospitality in a constructive and engaging format. It does not advocate a political position and seeks to encourage dialogue around public services.

This form seeks consent for your child to utilize Zoom for distance-based, virtual student participation in the Master Chefs event. Please review Zoom’s privacy terms and conditions carefully before registering your child: http://zoom.us/terms and http://zoom.us/privacy. If you do choose to register for Zoom, you will be asked to provide basic information, not limited to name and email address.

It is necessary that parents/guardians specifically authorize their child to be included in this event. While supervision for this event will be furnished by SDCOE and event partners, parents/guardians are hereby advised that such supervision will occur only during the activity duration stated above. Although we will take every precaution to assure the welfare and safety of all children participating in this activity, it is important to understand that SDCOE and partners assume no liability whatsoever in case of injury or accident, as noted in further detail in Section 4 - Parent/Guardian Participation Agreement to Hold Harmless.

VIDEO/AUDIO RELEASE
The participant’s parent or guardian acknowledges that SDCOE and event partners reserve the right to photograph/video record participant during the activity designated in this notice for future use, including but not limited to: publicizing, advertising, promoting, or marketing the aforementioned program and/or any portion thereof.
PARENT/GUARDIAN PARTICIPATION AGREEMENT TO HOLD HARMLESS:
Parent/Guardian or his or her heirs, executors, administrators, successors, and assigns agrees to hold harmless, defend, and to indemnify SDCOE and partners, accept any and all responsibility for loss or damage to any person or entity, including SDCOE, and to indemnify, hold harmless, and release SDCOE, its officers, agents, and employees, and event partners from and against any actions, claims, losses, damages, liabilities, disabilities, or expenses for injury to or death of any person(s), or damage to property relating to or arising from his or her participation in the activity designated in this notice that may be asserted by any person or entity, including Parent/Guardian or his or her heirs, executors, administrators, successors and assigns. Parent/Guardian’s obligations as discussed herein shall apply whether or not there is concurrent negligence on SDCOE’s and partners’ part but to the extent required by law, excluding liability due to SDCOE’s and partners’ conduct.

By completing this form in its entirety, I HEREBY CERTIFY AND ATTEST that I am the legal parent or guardian of the student mentioned hereunder and have the requisite authority to consent to my student's participation in SDCOE’s distance-based virtual student event, as described herein, on careers in public service.
