Instructions: Please fill out this 4K enrollment form for the 2024-2025 school year. Submissions will be time-stamped on a first-come first-serve basis.
Besides completing this form, you will also need to submit the Required Documentation (listed below). Please email or drop-off the other required documentation to finish the enrollment process after you click "SUBMIT." You will have one week to email or drop-off the required documentation to finish the enrollment process.
REQUIRED DOCUMENTATION includes the following: (please send copies only; submitted documents will be shredded after verification)
1. Copy of parent/guardian photo ID
2. Copy of original Birth Certificate
3. Copy of proof of residency (which may be a copy of any of the following: a piece of mail that was run through USPS, a notarized letter of property owner stating the family lives there, official change of address from USPS, or a letter from your bank)
4. $20.00 material fee (this can be cash, check, or online payment via Skyward. More information to follow.)

After the registration process is complete, you will receive a confirmation email that your documentation has been received, along with the timestamp of your registration. Your timestamp will be the date/time that you hit "submit" at the end of this form.
