To ensure clarity and student safety, ALL FAMILIES INVOLVED IN A CARPOOL MUST SUBMIT A REQUEST.

Requests submitted by NOON on Wednesday, August 19 will be processed in time for the start of school.

NOTE: You also must fill out this form if your child will be picked up after school by a day care OR after-school activity provider.
