By registering for this clinic and entering my name here as an electronic signature, I commit to purchasing an NCR-USAV membership, requesting a USAV-approved background screening (if 18 years of age or older), and completing SafeSport training prior to this clinic. In addition, I will complete all required online officiating training prior to the clinic date. *
