This application form is for a Family Discount for Registration Fees where a family has 3 or more players registered to play with the Mt Colah Football Club in the 2022 season.
- The family discount is up to the value of 15% of the total standard registration fees paid by all players in a single family.
- To be eligible for a family discount all players must be members of the same family and reside at the same address.
- Completion of this application form on its own is not approval for a family discount.
- Mt Colah Football Club reserves the right to reject this application.
- The family discount is applied to one of the outstanding invoices or paid to the person who pays the fees.

Once this application has been submitted the Treasurer will consider the application and advise if a Family Discount is to be approved.
