This form is both for Annual USA Curling Insurance Registration and to determine which dues you need to pay for the 2023-24 season. You will need to pay dues on our Square site after completing this form.
All members must be INDIVIDUALLY registered and must sign the waiver form before curling.
Lockers may be shared with your team. They are an optional add-on cost to your member dues.
