Student Alternate Departure Form
Parents must log-in to their Parents Web account (https://logins2.renweb.com/logins/ParentsWeb-Login.aspx)  and review/update their students' pick-up and emergency contact information that is currently listed in FACTS SIS. An Alternate Departure Pass will be printed for the students to attach to their backpack using the information that is entered in FACTS SIS.  **IMPORTANT:  Information and instructions on updating this in FACTS Parents Web were outlined in the email sent to all parents.  Please refer to that email for instructions on how to review/update pick-up/departure and emergency contact information.**

Using the link provided in the email, I have reviewed/updated my students' alternate pick-up/departure information as well as my students' emergency contact information through FACTS Parents Web.  I understand FACTS SIS is the only way to update alternate pick-up/departure and emergency contact information for my student. I have fully completed the review/update process in FACTS Parent Web for my students. *

I understand that my child will attach the Alternate Departure Pass to their backpack. If it is lost, my child will not allowed to leave campus with anyone other than the parent/guardian or through an approved notification from the front office.  Fees will be assessed for this service. I also understand that a cost of $5.00 will be charged to replace the Alternate Departure Pass.

Students leaving ICCS may walk to the address listed on this form.  These students will be allowed to leave at 3:15 p.m.

I understand that my child will attach the Walk Home Pass to their backpack. If it is lost, my child will not be allowed to leave campus with anyone other than the parent/guardian. My child will be registered into Extended Care until I am able to come retrieve him/her.  Fees will be assessed for this service. I also understand that a cost of $1.00 will be charged to replace the Alternate Departure Pass.
