2021 Disable the Label Family Fun Fest VENDOR BOOTH Form
The 7th Annual Disable the Label Event is being held on October 23, 2021 from 9a-12noon at the back campus of Our Lady of the Lake Livingston in Walker, LA. This annual event will look a little different this year as it will be in a new location and will not have the 5K event portion, but will be growing in the community awareness event that has always been held as the highlight of our families that attend. This year's Family Fun Fest will be bigger than ever and will be held outdoors with tents, vendor participation, games, a 1 mile FUN walk around the beautiful OLOL Livingston Lake, and feature more interaction with the community than ever before.

We have some changes to our VENDOR BOOTH areas for this year's event. We would love to have you join us in creating a safe and fun interactive event that supports our patients and their families along with rallying the support of our community in one amazing event. The VENDOR BOOTH participation selections are below. Once you complete the form, you may mail a company check made Payable to Launch Therapy Center and mailed to 601 Hatchell Lane, Denham Springs, LA 70726 or you may click this link to pay by Credit Card ( https://checkout.square.site/merchant/4408V8EMMVJB3/checkout/Q34SCUBEUB6XZJZ7HHBJEU2M  ) in the amount listed. Once your payment has been received, you will be emailed a confirmation and participation info for DAY OF for this event including Set up times, items needed to bring, and can/can't do list for this event.

All Vendors signed up by the deadline will be promoted on our Launch FB page, Event FB page, and within the Event signage near the vendor area. Each vendor will get 4 individuals to attend to work the event only. These will get access to all event spaces for the duration of the event. You must check in ALL individuals that will work the Vendor Booth at the Vendor Check-In on the day of to sign in and get their designated wrist bands. NO ONE will be allowed in the event area without doing this first. Any additional than 4 will be required to register for the event and pay the corresponding registration fee. This is a safety measure put in place specifically for this event type for our therapy patients and families.
