I hereby give permission for my child to participate with the Delaware Sharks Field Hockey Club in preparation for, and during, the 2020 Sharks Tournaments at John Dickinson High School. I certify that she is physically fit and that Delaware Sharks FHC, LLC, its directors, and coaches , as well as members of John Dickinson High School staff, will not be held responsible for any accidents or injury during the course of the tournament. If any emergency care is required, I give permission to Delaware Sharks FHC and their employees to administer first stage emergency treatment. *
