This form is to enroll students for children and youth ministry programs at Bridge of Faith Community Church.  Enrollment is open to all children and students from birth-12th grade.

Transportation is available within 5 miles of the church for children over age 5. Children younger than 5 may be able to ride if an adult (age 18 or older) is riding with them. Beginning October 18, 2020, transportation will be available only to Sunday morning service which begins at 10:30 am.

There will NOT be transportation for Wednesday nights. Beginning February 3, we will have a class for 4 year olds-5th grade in addition to youth (Grades 6-12). An adult Bible study is also meeting at the church on Wednesday nights. Unfortunately, we will not have nursery available on Wednesday nights at this time.

Enrollment is required for all students under age 18 to ride the bus and to participate in Wednesday night Bible studies and activities.

As our response to COVID-19 and social distancing guidelines, we have made some adjustments to our wellness policy and the structure of our programs. We want to ensure that we are doing what we can to keep our children safe and healthy.  We have implemented temperature checks as students are getting on the bus or if they are dropped off at our campus.  We will be using no-contact infrared thermometers.  If your child has a temperature above 99.9 degrees, they will be asked to remain at home until they have been fever free (without the use of fever reducers) for 24 hours.  If your child has been sick, we ask that you keep them at home until they have been symptom-free for 24 hours.  If your child becomes sick while they are at church, we will ask you to come pick them up immediately. As guidelines and updates pertaining to COVID-19 are constantly changing, our guidelines and plans are subject to change as well.  We will do our best to communicate any changes to you as soon as possible.
