Thank you for taking the first step towards enrolling your child in a high-quality early learning program!

You may complete this application online or visit Vivian Adams Early Childhood Center or Dr. Katie Harper Wright Elementary School to pick up a printed version. No matter which application you complete, in-person screenings are required. You MUST submit the APPLICATION and the HIGHLIGHTED DOCUMENTS (#1, #4, #5, & #6) to be scheduled for an Enrollment Screening. UP TO DATE Documents #2 & #3 MUST BE SUBMITTED BEFORE YOUR CHILD CAN OFFICIALLY START AT VIVIAN D. ADAMS OR DR. KATIE HARPER-WRIGHT.

1. Child's Certified Birth Certificate
2. Current Physical Examination
3. Current Immunization Record
4. Income Verification for Parent(s)/Guardian(s) living in the child's place of residence
    (2 current pay stubs, SSI statement, TANF statement, W2 or 1040 form, Zero Income statement)
5. Two Proofs of Residency (apartment / house lease, mortgage statement, property deed, voter registration card,
    State medical card, utility bill - gas / water / electric, or Statement of Residency)
6. Valid Driver's License or State I.D. card for Primary Parent / Guardian

After items #1, #4, #5, & #6 are submitted, a member of the Vivian Adams/Dr. Katie -Harper-Wright staff will contact you to schedule an Enrollment Screening.

The following procedures are also required, but may be available during your scheduled screening appointment or during the school year:
1. Dental Screening
2. Vision & Hearing Screening
3. Dial-4 Language Screening

If your child has a Special Services referral or an IEP, please bring all related documentation to your scheduled, in-person screening appointment.

NOTE: Special considerations will be made for families experiencing homelessness or other emergency conditions.
