Sport Club Membership Packet
This form should be completed by ALL MEMBERS of a sport club. A Liability Release and Indemnification Agreement must be physically completed and submitted by all individuals prior to their participation in any club function. These paper forms will be submitted by club officers within 24 hours of receipt. All membership and waivers will be tracked by Club Secretaries and Recreational Sports administration.
