ALL match uniform size requests, training uniform and additional apparel orders must be submitted through this Google Form by the end of day on Friday, October 27.  Match uniforms, training uniforms and any additional apparel purchased will be distributed to players either the week of October 30th or November 6th.  

Payment (check) or proof of payment through online options (print out or electronic verification of payment confirmation) must be provided at distribution.
