This form is used to log your quilt in to Mikalas Quilting Longarm Queue.  Please note that this is a first in, first out queue and we will do our best to get your quilt back to you as soon as we possibly can.  Please do not bring your quilt to Mikala's - after receiving this form we will let you know when we are ready for you to bring your quilt in.   Thank you!   We look forward to quilting for you!

In the event while quilting for you we find it necessary to steam your quilt &/or the borders in order for them to lie flat, you agree to a steaming fee of $25. From time to time - quilting mishaps happen.  We will notify you and either we can make a necessary repair or can let you do it.  By submitting this quilting request form, you also agree to having your quilt photographed and shown on social media such as, but not limited to, Facebook and Instagram.  Thank you!   (Please type your name for your signature) *
