Elementary Pre-arranged Absence Form
Thank you for making us aware of your student's upcoming absence. We ask that families do their best to limit extended absences, as attendance at school is a central contributor to a student's success.  We also understand, however, that life happens, and there are times when these types of absences are unavoidable.
We appreciate your open communication and partnership to support your student’s wellbeing and success.

Upon completion, your responses to this form will be routed to our attendance office as well as to our middle school administrators. Please use separate forms for each student.

I understand that my student is responsible to make up academic work missed as a result of this absence. Teachers are not expected to provide additional lesson materials, or virtual/on-line instruction, to my student during this absence. Teachers will be available during regularly scheduled working/school hours to support my student upon their return.

I understand that although this absence may be marked as “excused” in the school attendance system, it may still cause my student’s overall attendance rate to drop below the established desired threshold.  This means that an attendance letter may be sent from the attendance office.
