HOSA Membership Form 2023-2024
Hello,

If you are interested in joining HOSA in Fall 2023, all you have to do is pay the $35 membership fee and fill out this form.

Thank you for wanting to become a HOSA member! Our organization provides:
- Various pre-health related workshops every month 
- An opportunity to compete in the state and international level
- Volunteering Opportunities 
- Clinical Hours
- Socials to meet other club members 
- Fundraising events to pay for competition
- Mentor program for new members who will be assigned to returning members
- Mentor program in the spring semester for pre-medical students applying in the upcoming cycle who will be assigned to current medical students for help in applications, essays, interviewing, and advice

Membership fee:
$35
Pay through Venmo (@UTD-HOSA) or PayPal using the PayPal button on our website.
