A.S. Write-In Candidate Application Request Form
Hello!

To request an application to be a write-in candidate in the 2021 A.S. Elections, please complete this form in full.  

Once you submit this form, a Google form link to a candidate application and supporting materials will be sent to you within twenty-four hours via email.  

All applications to be a write-in candidate in the 2021 A.S. Elections are due Friday, March 19, 2021, by 12:00 p.m. PDT.

NOTE:  Completing this request form does NOT confirm your candidacy to run in the A.S. Elections. You must submit your application form by the deadlines above.
