We are trying to make it easier for you to share information with us about your child. This form requests information about ; Contact Details, Medical Information and National Identify and Ethnic background. It is an electronic version of a paper copy that has been shared in previous years.
In order to reduce the amount of paper back and to between school and home, I ask that you complete the form electronically.
