As the COVID-19 situation continues to evolve, we’d greatly appreciate you completing this form if you or anyone in your household may be traveling in March 2020 OR recently traveled in February 2020. This information will be collected by the Head of School's Office.

Additionally, we ask that members of our community who plan to travel refer to the CDC’s travel advisories and adhere to mandatory quarantines upon return, if applicable.  This survey will be open through the beginning of Spring Break, March 13.
