Responsible Use Agreement for USC Chromebook/Tablet Use--Parents/Guardians: Review the Responsible Use terms. Then enter your name and the name of your student(s) to acknowledge your understanding of how your students are expected to handle their USC device safely and respectfully.
Parent/Guardian Responsibilities
My son/daughter has been issued a device (Tablet or Chromebook) by District 2134 to enhance his/her education. I agree that my son or daughter is to use the device in accordance with the USC Student Handbook, the USC Bullying/Harassment Policy, and the rules outlined below.
● I understand that my son/daughter is responsible to do assigned work even if he/she is without a Tablet/Chromebook.
● I agree not to send a family-owned, personal computer or tablet to school for use during the school day. If a non-school device is ever needed at school, I understand that permission must be granted by a district principal.
● As much as possible, I will supervise my student’s use of the Tablet/Chromebook at home and remind him/her to charge the device nightly.
● I will report to the school any problems with the device. I will not attempt to repair the Tablet/Chromebook or arrange repair for it on my own.
● I recognize that the device is District 2134 property, is monitored, and may be reviewed at any time.
● I know that disciplinary action will follow if my son or daughter violates USC’s established rules and policies about device and Internet use.
● I understand that use of the device may be restricted to encourage more focused use of the Chromebook or Tablet.
● I will make sure that my son/daughter returns the Tablet/ Chromebook when requested to do so and upon withdrawal from United South Central Schools.
● I am responsible for the cost of repairs and replacement of the Tablet/Chromebook. (Note: An insurance policy is available for purchase through the School District.)
