Dear Parent or Guardian,

 Your child has been chosen to participate in the Youth Empowered to Prosper (YEP) Program mentoring program called, “Young Women Who Win (YWWW)!” In the program, your child will be matched with a female mentor for the duration of the school year. The volunteer mentor will meet with your child on the school grounds and/or virtually, for four (4) hours per month or approximately one hour per week.

The volunteer will act as a mentor and source of encouragement, especially around academic goals, and personal development. The relationship and activities between your child and the mentor will be closely monitored and supervised by the YEP Program Coordinator, Alexandra Alexis. Ms.  Alexis will gather information on your child’s school performance and attendance to be used strictly for the purposes of evaluating the program and will be kept confidential.

The mentors that volunteer for our program have been thoroughly screened and investigated. We respect your role as a parent/guardian and will provide you an opportunity to meet with the mentor and be involved in the development of the relationship between the mentor and your child. If you would like your child to participate in the program, talk about it with her. If she is comfortable with the idea of having a mentor, please grant her permission by signing below.

I understand the nature and the guidelines of the school's mentoring efforts and reserve the right to withdraw from the program at any time.

Student Pledge

 By choosing to participate in the Youth Empowered to Prosper (YEP) Mentoring program, I, the Student Match, understand that I must:

1.       Follow all rules and guidelines as outlined by the program coordinator, mentor, mentee training, program policies and this contract.

2.      Have a positive attitude and be respectful of my mentor and other mentees.

3.      Learn new things; be responsive to suggestions and constructive criticism.

4.      Make a one-year (school year) commitment to being matched with my mentor.

5.      Meet for a minimum of four hours per month with my mentor.

6.      Be on time for scheduled virtual meetings or contact my mentor ASAP or at least 24 hours beforehand if I’m unable to make a meeting.

7.       Not participate in activities with my mentor unless it is approved and known of the YEP program coordinator and your child’s school.

8.      Openly communicate with the program coordinator and discuss your relationship with your mentor.

9.      Inform the YEP program coordinator of any difficulties or areas of concern that may arise in the relationship, or that might affect my ability to participate fully.

10.   I will not travel with my mentor.

11.    Participate in scheduled events and/or meetings for mentees.

12.   Participate in YEP’s protocol for data collection and evaluation of the mentoring program including but not limited to report cards.

13.   Participate in the mentoring closure process/graduation celebration when that time comes.

14.   Notify the program coordinator if I have any changes in address or phone number.

I understand that upon match closure, future contact with my mentor is beyond the scope of the YEP Mentoring program and may happen only by the mutual consensus of the mentor, the mentee(s) and parent/guardian. I agree to follow all the above stipulations of this program as well as any other conditions as instructed by the YEP program coordinator at this time or in the future.

Parent Authorization to Participate and Information Release

By signing this consent form, I agree that my daughter has my permission to participate in the Youth Empowered to Prosper Program. I understand that my child will meet in person and/or virtually with a mentor weekly who has been background checked and whose relationship with my child will be monitored by the Youth Empowered to Prosper program coordinator. I agree to encourage my student to attend meetings, meet with their mentor, and otherwise fully partake in the workshops, field trips, college tours, etc. the Program provides.

 I understand that, as a condition of receiving such mentorship, I hereby authorize my child’s school to release to Youth Empowered to Prosper (on a continuing basis) information about my student, including information and documents relating to his/her enrollment status, my academic standing, grades and academic activities, personal statements, advisor statements, financial information and any other information the Youth Empowered to Prosper program may request. I understand that some or all of the information released to Youth Empowered to Prosper may be personally identifiable information from my child’s student records pursuant to the Family Educational Rights and Privacy Act of 1974 (FERPA) or which may be otherwise protected under other applicable privacy laws. Your child’s information will not be released to any other organization.

 Parental Consent for Release of Photograph & Information

I hereby grant my child’s participation in approved Youth Empowered to Prosper (and those working on its behalf) in which she may be photographed or videotaped (the property) from time to time.

For valuable consideration received, you hereby irrevocably grant to YEP and its affiliates, other partnering organizations, perpetually, exclusively, and for all media throughout the world (including print, non-theatrical, my child’s photograph, video image, writing, voice recording, name grade level, school name, participation in activities and sport, diplomas and awards received, playbills, school productions, website, social media sites, internet, etc. and any other electronic medium presently in existence or invented in the future), the right to use and incorporate (alone or together with other materials), in whole or in part, photographs or video footage taken of you as a result of your participation in approved activities of YEP solely for the purposes of YEP promotional material and publications, and waive any rights of compensation or ownership thereto. You hereby agree that you will not bring or consent to others bringing claim or action against YEP on the grounds that anything contained in the Property, or in the advertising and publicity used in connection herewith, is defamatory, reflects adversely on you, violates any other right whatsoever, including, without limitation, rights of privacy and publicity.  You hereby release YEP, its directors, officers, successors and assigns from and against any and all claims, demands, actions, causes of actions, suits, costs, expenses, liabilities, and damages whatsoever that you may hereafter have against YEP in connection with the Property.

Parent/Guardian, by signing this electronic document with your First and Last Name, you have acknowledged you have read the entirety of this form and have authorized your child to participate and have consented to the release of photograph & videography.
