The preschool classes will be traveling to the Peoria Zoo in Peoria, Illinois on Friday, May 24, 2024. Please complete this form by Friday, April 26 to let us know if you give permission for your student to participate.
If you do not complete this form, your student will not be able to attend the field trip. There will NOT be PreK class on this day if you choose not to attend the field trip.
PreK field trips require an adult to accompany the student. No other children are permitted to attend. Everyone will be transported by bus; however, if you want to drive your own personal vehicle with your child, this will be permitted. Please understand that you will NOT be reimbursed mileage or parking fees if driving on your own.
The bus will depart promptly from Northwest Elementary School at 8:30 a.m. and return at approximately 2:30 p.m. Please arrive to the school by 8:15 a.m. so we can be sure to leave on time.
There is NO cost for your child and his/her accompanying adult to participate in this field trip. All admission fees are paid for by the district and the prek grant.
Please bring a lunch for yourself and your child. Please do not bring any glass or breakable containers. Straws are also not permitted in the zoo.

Las clases de preescolar viajarán al zoológico de Peoria en Peoria, Illinois, el viernes 24 de mayo de 2024. Complete este formulario antes del viernes 26 de abril para informarnos si da permiso para que su estudiante participe.
Si no completa este formulario, su estudiante no podrá asistir a la excursión. NO habrá clases de PreK ese día si decide no asistir a la excursión.
Las excursiones de PreK requieren que un adulto acompañe al estudiante. No se permite la asistencia de otros niños. Todos serán transportados en autobús; sin embargo, si desea conducir su propio vehículo personal con su hijo, esto estará permitido. Por favor, comprenda que NO se le reembolsarán el kilometraje ni las tarifas de estacionamiento si conduce por su cuenta.
El autobús saldrá puntualmente de la Escuela Primaria Northwest a las 8:30 a. m. y regresará aproximadamente a las 2:30 p. m. Llegue a la escuela antes de las 8:15 a. m. para que podamos asegurarnos de salir a tiempo.
NO hay ningún costo para que su hijo y su adulto acompañante participen en esta excursión. Todas las tarifas de admisión son pagadas por el distrito y la subvención de prek.
Por favor traiga un almuerzo para usted y su hijo. Por favor no traigas recipientes de vidrio o rompibles. Tampoco se permiten pajitas en el zoológico.
