As a parent or legal guardian of a minor attending the 2021 Pursuit volleyball tryouts, practices, trainings, clinics and camps, I understand that Pursuit volleyball, Cape Sportsplex, Osage Center, Shawnee Park Center and Cape Girardeau Parks and Recreation are not liable for any injury, illness or death during participation of volleyball activities. If, during the course of my child's activities at volleyball, he/she should become ill or sustain injury, I hereby authorize Pursuit Volleyball instructors to obtain emergency care. I have agreed to and assume all risks related to COVID-19 and Pursuit has my signed waiver on file. I will assume financial responsibility for bills incurred through my insurance company. I also authorize Pursuit Volleyball to share pictures of my child and their name on Pursuit social media promotions. *
