Pecatonica CUSD 321 Official Opt-In or Opt-Out Form
This form will determine whether your student experiences in-person or remote learning to begin the 2020-2021 school year.  

Please fill out ONE survey for EACH STUDENT that is enrolled at Pecatonica CUSD#321. Survey will close at 4:00 pm on Monday, August 10th, 2020.

Please note that if you opt for remote learning that this will remain your selection until AT LEAST the conclusion of the first quarter, unless approved by administration.

For my student, I choose to have him/her, ________, with the understanding that in-person instruction will require students to wear masks and that social distancing will be enforced, as possible, AND the understanding that remote learning will be substantially more rigorous (grading) and thorough (scope & pace) compared to the remote learning process last spring and will include attendance requirements per return to school plan with remote option. *

Transportation guidance allows for 50 people to ride the bus with social distancing, to the extent possible and face masks required. With this in mind, please provide which transportation option your student will need to have for in person learning.
