GUARDIAN CONTRACT: As a parent/guardian I acknowledge that I have read the WYC Comp Team Information Packet for the 2019-2020 Season.  I understand and endorse the expectations and requirements set forth in that packet for my child to be a part of the WYC Comp Team.  I have read the financial expectations for being a part of the WYC Comp Team.  I understand that if my child is selected for the team, I will be responsible for paying these expenses for my child to be allowed to participate.  I recognize that as a result of participation, medical treatment on an emergency basis may be necessary and team personnel may be unable to contact a parent for consent in an emergency medical situation.  I do hereby consent in advance to such emergency care, including hospital care, as may be deemed necessary under the existing circumstances.  I understand that by the very nature of the activity, cheerleading and gymnastics carry a risk of physical injury.  No matter how careful the participant and coaches are, how many spotters are used, or what landing surface is used, the risk cannot be eliminated.  The risk of injury includes minor injuries such as muscle pulls, dislocation and broken bones.  The risk also includes catastrophic injuries such as permanent paralysis or even death from landing or falling on the back neck, or head.  I understand these risks and will not hold the Beaverton School District, Westview High School, Westview Youth Cheerleading, Westview Youth Comp Cheer, or any of their agents responsible in the case of accident or injury at any time. *
COVID-19 RELEASE: The novel coronavirus (“COVID-19”), has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. While rules, guidance, and personal discipline may reduce this risk, the risk of serious illness and death does exist. Westview Youth Cheer and Oregon Dream Teams cannot completely mitigate the transfer of communicable diseases like COVID-19 especially when involved in cheerleading. Participation in cheerleading includes possible exposure to and illness, injury, or death from infectious diseases including COVID-19. In consideration for providing my child the opportunity to participate in cheerleading events, both my child and I voluntarily agree to waive and discharge any and all claims against WYC and ODT and release it from liability for any exposure to or illness or injury from an infectious disease including COVID-19, including claims for any negligent actions of the program or its employees or agents, to the fullest extent allowed by law, for myself, my child, our estates, our heirs, our administrators, our executors, our assignees, and our successors. I also agree to release, exonerate, discharge and hold harmless WYC, its coaches, the individual members thereof, and all officers, agents, employees, volunteers, and representatives from all liability, claims, causes of action, or demands, including attorney fees, fines, fees, or other costs (e.g. medical costs) arising out of any exposure to or illness or injury from an infectious disease including COVID-19, which may result from or in connection with my child’s participation in cheerleading. I further certify and represent that I have the legal authority to waive, discharge, release, and hold harmless the released parties on behalf of myself and the above-named student. I certify that I have read this document in its entirety and fully understand its contents. In exchange for the opportunity to participate in cheerleading, the above-named student and I freely and voluntarily assume all risks of such hazards and notwithstanding such, release WYC from all liability for any loss regardless of cause, and claims arising from the student's participation in cheerleading. *
